Please review the summary of each document or form before submitting it to our office.  Make sure all required fields have been completed before submission.  Knowingly or intentionally falsifying information submitted to our office may subject you to penalties prescribed by law under the Pa. Crimes Code Section 4904 relating to the crime of Unsworn Falsification. 

Diversionary Programming Application - Accelerated Rehabilitation Disposition/Probation without Verdict/Felony Diversion Program

This form should only be filled out and submitted to the District Attorney's Office if you have been advised by your attorney, or if you do not have an attorney and have been instructed to apply for this program by the Assistant District Attorney during your preliminary hearing.  

Private Criminal Complaint    Private Criminal Complaints are scheduled by appointment only, please call 570-327-2456 to schedule your appointment.

This form should only be filled out and submitted to the District Attorney's Office after reporting the incident to your local police department and being instructed by the police to contact the District Attorney's Office to file a Private Criminal Complaint.  Please do not sign and date this from.  You must appear in person at the District Attorney's Office with legal photo identification where a notary will witness you signing the complaint.  You must make an appointment to review/file your complaint in person with a paralegal.
